A. There are a few options:

The most complete and reliable source would probably be the DD Form 214 (DD 214) “Long Form”, Certificate of Release or Discharge from Active Duty — a document issued upon a military service member’s retirement, separation, or discharge from active duty. The DD 214 “Short Form” contains only partial info and leaves out a lot of things that would be of interest for the plaque.
Prior to 1950 it would be the “WD AGO” (War Department Adjutant General’s Office) Forms or the NAVPERS (Naval Personnel) discharge documents.
If you don’t already have these forms, they may be acquired from the National Archives at archives.gov/veterans/military-service-records/standard-form-180.html. This could take some time, so there are sources online that can help expedite the process.
Photos of the veteran showing ribbons, badges, & patches are also a good source, though sometimes incomplete.
Other references, such as photos letters, or family knowledge can also help.

A. The accuracy of the information is your responsibility, as I won’t have time to do the research. The plaque will display the info you send. Since the plaque is a memento for veterans and their families, and not an official document, some honest errors may happen. Hopefully no deliberate false claims or exaggerations constituting “stolen valor” will occur.

A. The larger the image the better.

An image around the size of an 8” X 10” photo should work well enough. A smaller digital image may also work if it’s resolution is high enough. We can enhance images to a point, but it really depends on the quality of the image we start with.

A. Since I personally create each plaque design, design phase time may vary depending on the work flow. Normally the plaque should ship within 1 week after your final approval and authorization to print is received.

A. You will be sent a proof for final review before the plaque is produced. It is your responsibility to closely check every element on the proof for accuracy, including spelling. Once you are satisfied that the plaque meets your approval, you will reply with an authorization to print, at which time the production process will be initiated.

A. Normally I can supply the images needed. In cases of rare unit patches or insignia you may need to supply an image.
